You kids? What are we, 6 compared to you? I agree with Sal and Ryouta. The Republic Forces couldn't get to the Star Forge while it was still mass-producing ships and droids. The Jedi mainly took on the Dark jedi within, while Revan went for Bastila and Malak. I don't see any mistake Malak made, he lost in a duel with Revan. Plain and simple.

The mistake Malak made was his confidence that he had in fact killed Revan. Instead of making sure the job was done by annihilating his flag ship he only destroyed the bridge.
